New Patient Information
Everything you need to know before your first visit with Dr. Chavarria.
What to Bring to Your Appointment
- Photo ID and insurance card
- List of current medications and dosages
- Copies of any prior MRI, CT, or X-ray images themselves (on CD or digital format) — not just the radiology reports
- Prior medical records related to your spine condition if available
- List of questions you want to ask
- A family member or trusted person is welcome to accompany you
Important: Please bring copies of the images themselves — not just the imaging reports. Arriving without the actual images may delay your care, since your spine cannot be fully evaluated from the reports alone.

History & Examination
Dr. Chavarria will review your symptom history, prior treatments, and perform a focused neurological examination to identify the source of your symptoms.
Imaging Review
Your MRI and other imaging will be reviewed in detail. Dr. Chavarria will walk you through what the images show and how it relates to your symptoms.
Discussion of Options
You will receive a clear explanation of what is causing your pain and all available treatment options — conservative and surgical. There is no pressure to proceed with surgery.
Next Steps
If surgery is recommended, you will receive a detailed explanation of the procedure, risks, benefits, and recovery. If additional workup is needed, orders will be placed.
Frequently Asked Questions
Do I need a referral to see Dr. Chavarria?
Many insurance plans do not require a referral for orthopedic or spine specialist visits. We recommend checking with your insurer. If your plan does require a referral, your primary care physician can provide one.
What should I bring to my first appointment?
Please bring your photo ID, insurance card, a list of current medications, and copies of any imaging you have had (MRI, CT, or X-ray) on CD or digital format. Be sure to bring the actual images themselves — not just the imaging reports — as your spine cannot be fully evaluated without them.
Will Dr. Chavarria recommend surgery at my first visit?
Not necessarily. Dr. Chavarria's approach is conservative-first. He will review your history and imaging, discuss all available options, and recommend surgery only when it is clearly the best path forward for your specific situation.
Does Dr. Chavarria accept my insurance?
We accept most insurance plans. Coverage varies from plan to plan, so we recommend checking with your insurer to confirm your coverage before your appointment.
How long will I wait before getting an appointment?
Routine consultations are typically available within about one to two weeks. If you have urgent symptoms — rapidly worsening weakness, numbness, or bowel or bladder changes — tell us and we'll prioritize getting you in sooner.
Can I get a second opinion from Dr. Chavarria?
Absolutely. Dr. Chavarria welcomes patients seeking a second opinion and will provide an honest, independent assessment of your diagnosis and treatment options.
How do I prepare for spine surgery?
Pre-operative instructions are provided at your surgical planning appointment and may include stopping blood thinners, completing pre-surgical lab work, and arranging transportation home. Smoking cessation is strongly encouraged as it significantly improves fusion rates.
